Resident Care Director

The Warde Health Center located in Windham NH is seeking a Resident Care Director for our Supported Residential Care unit. This position requires an R.N License plus experience in an Assisted Living &/or other LTC environment. Candidate must have the ability to work collaboratively with peers as well as effectively with residents, family members and personnel under his/her direction. This position reports directly to the Warde Health Center Administrator. The Resident Care Director is responsible for day-to-day operations of the community.   Knowledge of Assisted Living State Regulations as well HCBC processes is required. Census management and admission over-sight is also a part of the responsibilities. The ideal candidate will also embrace and uphold the Mission of the Warde Health Center.

Assistant Administrator

Resident Assessment Coordinator

Hillcrest Terrace Retirement Community is seeking a conscientious and enthusiastic RN to coordinate the clinical and assessment protocols for our Health Center admissions, discharges, care management and transitioning of residents in and out of the community and throughout the continuum of care provided at Hillcrest Terrace.
This individual will also be responsible for assessment, development and all areas of resident care plans (including updates) necessary to provide for positive outcomes.  Current licensure as a Registered Nurse in NH is required and a minimum of four years experience preferably in a geriatric setting, two years of administrative experience in a residential or health care facility, and familiarity with MDS. This is a full-time position, 30 hours per week with a flexible schedule to accommodate resident and family availability.
